HealthONE's Northwest Clinic is located at 5044 West 92 Avenue in Westminster just north of the intersection of US36 and Sheridan Avenue on the southeast corner of 92nd and Sheridan in the Albertson's Shopping Center.
Occupational Medicine clinic hours are 7:00 to 5:00 Monday to Thursday and 8:00 to 5:00 Fridays.
After hours care is provided at North Suburban Medical Center at 9191 Grant Street.
We offer comprehensive and cost effective:
- Injury evaluation and treatment by Level II accredited physicians.
- On-site care coordination / case management.
- Physical examinations for DOT, pre-hire, and annual.
- Drug and Breath Alcohol testing.
- Treatment for blood-borne pathogen exposures
- Comprehensive rehabilitation services.
